Patient Assessment and Management:
Conduct comprehensive assessments of inpatients with diabetes.
Develop, implement, and evaluate individualised care plans in collaboration with the multidisciplinary team.
Monitor blood glucose levels and adjust diabetes treatment plans, including insulin therapy and other medications, based on clinical protocols and patient needs.
Diabetes Education and Support:
Provide education to inpatients and their families about diabetes management, including blood glucose monitoring, medication administration, dietary management, and lifestyle changes.
Offer psychological support to patients to help them cope with their diagnosis and treatment.
Insulin and Medication Management:
Administer insulin and other diabetes-related medications.
Educate patients on the use of insulin pumps and continuous glucose monitoring systems, if applicable.
Adjust insulin regimens in response to patient needs and changes in their clinical condition.
Prevention and Management of Complications:
Identify and manage acute complications of diabetes, such as hypoglycemia, hyperglycemia, and diabetic ketoacidosis.
Work to prevent long-term complications through effective inpatient management and patient education.
Team Coordination:
Collaborate with doctors, dietitians, pharmacists, and other healthcare professionals to ensure comprehensive diabetes care.
Participate in ward rounds, case conferences, and multidisciplinary team meetings.
Liaison and Communication:
Act as a liaison between the diabetes team and other hospital departments to ensure continuity of care.
Communicate effectively with primary care providers and community diabetes services for seamless discharge planning.
Staff Education:
Provide training and ongoing education to ward staff and other healthcare professionals on diabetes management and best practices.
Develop and deliver in-service training programs and educational materials.
Professional Development:
Keep up-to-date with current research and developments in diabetes care.
Participate in continuous professional development activities, such as attending relevant courses, conferences, and workshops.
Documentation:
Maintain accurate and comprehensive patient records, ensuring documentation meets legal and professional standards.
Document all aspects of patient care and communication in the patient’s medical record.
Audit and Quality Improvement:
Participate in clinical audits and quality improvement projects aimed at enhancing diabetes care within the hospital.
Contribute to the development and implementation of diabetes-related policies, protocols, and guidelines.
Data Management:
Collect and analyse data related to diabetes care and outcomes.
Use data to inform practice improvements and report findings to relevant stakeholders.
Support and Advocacy:
Advocate for the needs and preferences of patients with diabetes within the hospital setting.
Ensure patients’ rights and choices are respected in all aspects of their care.
Community Engagement:
Engage with patient support groups and community organizations to promote diabetes awareness and support for inpatients transitioning to home care.
Experience:
Clinical experience in diabetes management, preferably in an inpatient setting.
Experience in delivering patient education and staff training.
